The term of membership will be ended at December 31, 2038
Terms and Conditions
•
Payment of Annual Maintenance Fees within the same calendar year the payment is made.
•
All fees include Excise Duty Tax and VAT.
•
Associate of Membership is opened to the spouse, sons and daughters of the member. Sons and daughters must be under 20 years of age and maximum only 2 persons
•
In the unlikely event of a change, the Alpine Golf Resort- Chiangmai reserves all rights to amend the above rates with or without prior notice.
•
Fee for Member card and/or Membership Certificate substitute or replace shall always apply.
Document for application by individuals
1.
Four 1”x1.5” color photographs taken no later than 6 months.
2.
Two Certified copies of identification card or official identification card issued by the local authorities.
3.
Two photocopies of house registration form.
4.
A letter certifying your income, or a payroll slip or a withholding tax slip or the latest receipt for your annual personal income tax.
5.
A copy of your savings or fixed account statement for the past six months from a bank or financial institution. The first page of your bank book must show your name and account number. Alternatively, a copy of a primary credit card with the account that has been opened for more than one year.
6.
Foreigners need to include a copy of their passport and a work permit with a validity period of 2 years.
7.
Proof of ligitimate spouse or child will be required for Associate member.
Document for application by Coperate Representation
1.
Letter of authorization from the company confirming of the Corporate Member's Representative.
2.
Four 1"x1.5" color photographs taken no later than 6 months.
3.
Two certified copies of identification card or official identification card or passport of nominee of corpoate person issued by the local authorities.
4.
Two photocopies of house registration form of nominee of corporate person.
5.
A copy of savings or fixed account statement of corporate member's representative for the past six months from a bank and a copy of the first page of your bank book showing name and account number on it.
6.
A copy of Company’s Affidavit of Company Registration Certificate not older than 3 months and copy (ies) of Director's ID card.
